ACKNOWLEDGMENT and INDEMNITY

I acknowledge that I am making an application to play Softball in the Guelph Recreational Slo-Pitch League. I also acknowledge and understand that Softball is a sport and that there are inherent risks in playing and participating in a sport. By registering and further by participating in the playing of this sport I agree and accept the risks inherent to that sport and further understand and acknowledge that these risks could lead to personal injury.

For the consideration of being allowed to participate in the league, I hereby covenant and agree not to make any claim or commence or maintain any action against or proceeding against the Guelph Recreational Slo-Pitch League or its employees, officers, officials, agents, administrators successors or assigns or any person or corporation in which could claim against the Guelph Recreational Slo-Pitch League arising out of or in any way related to or connected with my participation in the playing, watching or coaching within the Guelph Recreational Slo-Pitch League.

Further, I hereby agree that I shall indemnify and save harmless the Guelph Recreational Slo-Pitch League and its employees, officers, officials, agents, administrators successors or assigns with respect to any and all claims which may be made against them by myself or my family.
